1) Your name
2) Your blog address (or your blogger profile is you don't have a blog - readers are also more than welcome)
3) If you would like to be added to the attendee sidebar (if you don't specify, I will not include you)
4) And if you do want to be on the sidebar, which name do you want me to refer to by (if you have a blogging name vs. your in real life name)
And keep checking back for updates prior to the event in case we need to make any last minute changes!
